2026-2027 PEAK Homes
Mentor Homes (Level 8)
The highest level — homes that excel in person-directed practices and help guide other homes.
At this level, homes have achieved full person-centered care competency by passing a comprehensive on-site evaluation that includes team, direct care, and resident interviews, practice observations and assessment of all 12 PEAK core areas for 2 consecutive years, demonstrating sustainability of practices over time. Level 8 homes must maintain mastery of at least 90% of the 85 individual person-centered practices across all cores through biennial evaluations while actively engaging in mentoring other homes in person-centered care implementation.

Sustained Person-Centered Care Home (Level 7)
Long-term leaders - homes that maintain excellent person-directed practices.
Level 7 homes have achieved full person-centered care competency by passing a comprehensive on-site evaluation that includes team, direct care, and resident interviews, practice observations, and assessment of all 12 PEAK core areas and have sustained this for two consecutive years.

Person-Centered Care Home (Level 6)
Fully person-directed - homes that meet all current standards.
Level 6 homes have achieved full person-centered care competency by passing a comprehensive on-site evaluation that includes team, direct care, and resident interviews, practice observations, and assessment of all 12 PEAK core areas. To reach this level, homes must demonstrate mastery of at least 90% of the 85 individual person-centered practices across all cores.

Level 5 Homes
Almost there - homes close to reaching full person-directed practices.
This level is achieved when a home has successfully passed 9-11 of the 12 PEAK cores and continues implementing person-centered care practices across additional core areas. Level 5 homes submit action plans and undergo in-person or Zoom evaluations to determine sustainability of previously passed cores, with the potential for a full evaluation assessing all 12 core areas that, if passed, advances them to Level 6.

Level 1 Homes
Getting started - homes beginning to make person-directed changes.
Level 1 homes develop action plans addressing at least 2 of the 12 PEAK cores and work on implementing all practices associated with their selected cores throughout the program year. Homes become eligible for Level 2 incentives by completing an in-person or Zoom evaluation, and passing at least 3 cores.

Foundation Homes
Learning stage- homes preparing to begin person-directed practices work.
The Foundation level is the entry point to the PEAK program, where homes complete the Kansas Culture Change Index (KCCI) evaluation tool and participate in structured learning activities with team members before implementing person-centered care practices. Homes must complete all required activities in "The Foundation" timeline and workbook to advance to Level 1.
