Heath Rath
Heath Rath, ACHA, is vice president and executive director of PACE at Midland Care Connection, where he leads operations across five centers serving 15 counties and more than 700 participants. He provides executive oversight of clinical operations, compliance, finance, workforce development and strategic growth.
Rath has spent his career building and scaling evidence-based, person-centered programs that enable older adults to age safely and independently in their communities, with deep expertise in the Program of All-Inclusive Care for the Elderly (PACE). He returned to Midland in December 2025 after serving as executive director of PACE KC, where he led the development and launch of Missouri's first new PACE program in decades. Earlier, he served as director of Midland Care Connection's Ann Street PACE Center, overseeing the launch of the Kansas City metro area's first PACE program.
Heath holds a Bachelor of Arts in Anthropology with a secondary major in Gerontology and a Master of Science in Gerontology from Kansas State University, with academic and professional experience in aging services, policy and program development.
